I know you are a doctor, so I am sorry if its silly to give you home medicine recommendations, but have you tried cleansing the infected area with a mild boric acid solution, such as the recommended eyewash recipe?
I use this at home for myself and pets for most skin problems. Its slightly drying, so I follow up with coconut oil.
